Pittsboro, NC - The Chatham County Public Health Department is now accepting nominations for the 2008 Community Health Partnership Award.
The award is for the local community partner who has advanced the practice of public health in Chatham County during 2008 by participating in partnerships that address prevention of disease, promotion of healthy lifestyles, provision of quality health services, and protection of the environment.
If you would like to nominate a partner, please download the form at www.chathamnc.org/publichealth or contact Vicky Guetgemann at 545-8391 for a copy.
All nomination forms should be returned to Vicky Guetgemann, Chatham County Public Health Department, P.O. Box 130, Pittsboro, NC 27312 or to vicky.guetgemann@chathamnc.org by the Friday, March 27th deadline.
